Receipt and Acknowledgement of Didactic Year Handbook
The Didactic Year Handbook is an important document intended to help the National Louis University Physician Assistant Program student become acquainted with and guided through the didactic year. The content of this handbook — including the policies reproduced in the Appendices — may be changed at any time at the discretion of the PA Program. In the event of a change, the student will be notified in writing. Please read the following statements and sign below to indicate receipt, acknowledgement, and understanding of this material: ● I have received a copy of the Didactic Year Handbook. I acknowledge that I understand the policies, rules, and information described in it, including the P.A. Student Employment Policy; PA Student Role, Non-Substitution, and Non-Staff Employment Policy; Incomplete Grade Policy; Leave of Absence Policy; Occupational Exposure, Injury, Illness, and Incident Reporting Policy; Student Dress, Professional Appearance, and Identification Policy; Confidentiality, Privacy, HIPAA, and Protected Information Policy; and Academic Grievance, Student Appeals, Harassment, Discrimination, and Non-Retaliation Policy, each reproduced in the Appendices, along with the Program Learning Outcomes, Professionalism Assessments, and B4.03 Summative Evaluation Blueprint. ● I understand the policies, rules, and information contained in the Didactic Year Handbook are subject to change at the sole discretion of the National Louis University Physician Assistant Program at any time. ● I understand that should the contents of the Didactic Year Handbook change in any way, for any reason, the Program will provide me with the current version and may require an additional signature from me to indicate that I am aware of and understand such changes. ● I have read and understand the material in its entirety contained within the Didactic Year Handbook, and I agree to abide by the rules, guidelines, and policies contained therein.
