NLU PA Program — Didactic Year Handbook
6. Tracking of Remediation Activities. Completion of remediation activities and student participation are documented in the Remediation Activity Tracking Log, supported by faculty meeting records, attendance records, skills or simulation performance, examination results, reflective assignments, and clinical or preceptor evaluations. 7. Reassessment and Outcome. Following remediation, the student undergoes the identified reassessment; the outcome is documented as successfully met, partially met, did not meet, requires additional remediation, or requires referral for further review, with a recommended next step. 8. Final Student and Faculty Sign-Off. The student is informed of the outcome and given the opportunity to acknowledge it. Faculty and Program leadership complete final sign-off verifying that activities, reassessment, outcome, any progression or committee decision, and secure filing of the record are complete. A refusal to sign does not delay implementation of a properly authorized decision; the refusal and method of communication are documented. 9. Secure Record Maintenance. Completed records are maintained as confidential student educational records in a secure NLU-approved electronic system or other secure Program-approved location, with access limited to authorized personnel with a legitimate educational or administrative need to know. 10. Record Retention. Records are maintained in accordance with applicable NLU institutional record- retention requirements and destroyed using secure NLU-approved methods when eligible. 11. Annual Audit Cycle. The Program conducts an annual audit of completed remediation records, evaluating whether required documentation, acknowledgments, authorizations, tracking, reassessment results, outcomes, and secure, retained records are present and complete, conducted by the Program Director, a designated administrator, the Program Manager, or another individual authorized by the Program Director. 12. Audit Findings and Corrective Action. When deficiencies are identified, the Program documents the deficiency and cause, develops and assigns corrective action with a completion timeline, verifies completion, and evaluates whether broader policy, training, or system improvements are needed. Significant or recurring findings are incorporated into the Program’s CQI process. 13. Annual Review and Oversight. Audit results are reviewed by appropriate PA Program leadership and, when applicable, the Student Evaluation and Progress Committee and/or PA Faculty and Staff Committee, and used to improve documentation practices, remediation and progression procedures, record security and accessibility, retention practices, and accreditation compliance.
