Sections XIII–XVI. Annual Remediation Record Audit and Record Retention (Program Use)

The Program conducts an annual audit of completed student remediation records, evaluated using the ARC-PA A3.16 Documentation Checklist above across the cohort, with findings, corrective actions, and a record-maintenance/retention tracking log maintained by the Program Director or designee. See the Procedure Statement below for the complete audit process.

Procedure Statement for Student Remediation, Sign-Off, Record Maintenance, and Annual Audit

1. Purpose. This procedure ensures that the NLU PA Program consistently documents, implements, tracks, evaluates, and securely maintains records of student remediation efforts and outcomes, supporting compliance with ARC-PA Standard A3.16. 2. Initiation of Remediation. When a student does not meet an established academic, clinical, professional, competency, or progression requirement, the responsible faculty member, course director, clinical education faculty member, or appropriate Program committee documents the concern and determines whether formal remediation is required. 3. Development of the Remediation Plan. The responsible faculty member or designated Program authority develops a plan identifying the deficiency and applicable standard, measurable remediation goals, required activities, a completion timeline, the reassessment method, responsible individuals, and the expected outcome or passing standard, which is then communicated to the student. 4. Student Acknowledgment and Sign-Off. The student reviews and acknowledges receipt of the remediation plan; the signature does not indicate agreement with the finding. If the student declines or is unavailable to sign, the communication is documented in the student’s record. 5. Faculty Sign-Off. The responsible faculty member signs to confirm the concern has been documented, the plan is appropriate and based on established requirements, the student has been informed, measurable outcomes are established, and the process will be documented. Additional sign-off is obtained when required for progression, clinical performance, professionalism, repeated deficiencies, or committee review.
